Role Description:

The ContinueCARE Liaison is responsible for educating the Dallas Ft. Worth medical community on all aspects of the services offered at the 4 Texas hospitals (located in Abilene, Midland, Odessa and Tyler) with emphasis on the referral and admission process. In addition, performs clinical assessments to determine appropriate patients for admission. This position reports to the Manager, Community Relations, Post Acute Services.

 

Job Functions:

 

● Complete assessment of patients being referred to determine patient’s clinical needs and appropriateness of admission.

● Ability to review and present admission analysis, referral analysis, needs of referral contacts, and how to meet these needs.

● Select most effective strategy (face-to-face, group presentation, direct mail, tours, workshops, etc.) for each contact. Exchanges ideas, information and messages readily and effectively, both in oral and written form, as required by the job. Provides patient updates to physicians, payers, and other referral sources.

● Complete face-to-face in-services to physicians, case managers and other professionals.

● Develop individual marketing plan and competitive analysis on time; able to identify opportunities to meet census budget; able to problem solve and implement measures to realize yearly census matrix; uses established reporting mechanisms and provides data and statistics as required. Completes physician profiles and territory/time management reports on time.

● Productivity: Identify potential growth, and current volume and importance to the hospital’s plan.

Education:

● Associate degree in Nursing or Respiratory Therapy, Bachelor’s degree preferred

● Current license to practice as RN or RT in the state of Texas

Prior Experience:

● 2 years Healthcare/Hospital experience with marketing or business development experience

● Knowledge of Dallas/Ft. Worth healthcare market

● Regional travel required
