Community Hospital Corporation Announces Six Hospital Winners of Quality Award

PLANO, TX – (May 5, 2014) – Community Hospital Corporation (CHC) is proud to announce six Texas hospital winners in the TMF® Health Quality Institute’s Texas Hospital Quality Improvement Award program for their dedication and commitment to improving quality of care.


Gold Award 

Memorial Medical Center — San Augustine, San Augustine, Texas – CHC management support

Mother Frances Hospital – Winnsboro, Winnsboro, Texas – CHC management support


Silver Award 

St. Mark's Medical Center, La Grange, Texas – CHC hospital

Yoakum Community Hospital, Yoakum, Texas – CHC hospital


Bronze Award 

Baptist Beaumont Hospital, Beaumont, Texas – CHC hospital

Bowie Memorial Hospital, Bowie, Texas – CHC management support


“We are proud of this recognition of quality care improvements,” said Wilson Weber, CHC executive vice president and COO. “I congratulate all hospital team members for their focus on providing exceptional and safe patient care throughout their facility and in everyday practice.


The Texas Hospital Quality Improvement Award program recognizes three tiers of winners for their performance on specific outcomes measures: Gold, Silver and Bronze. The program includes criteria for Inpatient Prospective Payment Systems Hospitals (IPPS), or Acute Care, and Critical Access Hospitals (CAH). The criteria include quality data reporting measures as well as readmissions measures.


This year, 33 Texas hospitals achieved the Gold Award, 45 hospitals earned a Silver Award, and 57 hospitals earned the Bronze Award.


The TMF® Health Quality Institute – the Medicare Quality Improvement Organization for Texas – operates the awards program in partnership with Texas Hospital Association and Texas Organization of Rural and Community Hospitals. This year’s winners will be recognized at a special ceremony in Austin on June 11.


TMF® Health Quality Institute’s goals are to: 

  • Improve the quality of care for patients through reliable delivery of inpatient and outpatient services and maximized use of effective care management strategies
  • Recognize high performing hospitals for their continued focus on quality improvement 
  • Align with statewide quality improvement efforts of the TMF Health Quality Institute, Texas Hospital Association and Texas Organization of Rural and Community Hospitals
